Scholarship Program

Each season CSAHA grants a limited number of scholarships toward the cost of a CSAHA fall season program. Please complete the secure, online application to apply for a scholarship.

Scholarships are awarded on a need basis, with player merit also taken into consideration.

All applicants must be current on all CSAHA fees, and registered for the program they wish to participate in.

All applications MUST include the supporting documents as requested before an application will be considered. Supporting documents are:

1. The first 2 pages of your most recent Federal Income Tax Return (Form 1040, 1040-A or 1040-EZ). If you do not file Federal Income Tax Returns, please attach your most current proof of income (i.e. State or Federal public support programs or Social Security income not subject to federal income tax. A recent pay stub is not acceptable unless it's confirmation of a significant change in annual pay and must be accompanied by the first 2 pages of your most recent Federal Income Tax Return.)
2. A current Annual Financial Statement. (Forms can be found online or obtained from your bank).
3. A letter of recommendation from your player's most recent NON-PARENT coach. This brief letter should touch on the player's attitude, teamwork, etc. It needs to be written by the coach providing it and must be current (do not use a letter from a prior season scholarship application). Other documents may not be substituted.

Applications that are not complete will not be considered.

Scholarships are awarded for the current season only. A new application must be submitted each season.

Please submit your application by July 31st. This helps CSAHA determine how to allocate the available scholarship funds.
